How to File Claims
If you or your beneficiary need to file a claim for Life and Accident Insurance Plans benefits, please contact HR Answers and speak with a Service Representative (please see the table entitled "Questions" under the "Life and Accident Insurance" section for information). If you or a covered dependent dies, a certified copy of the death certificate is required before death benefits can be paid. You will also be required to provide satisfactory evidence of a covered loss under the AD&D Insurance Plan.
